Continuation Costs
Following the first 6 weeks, if your dancer would like to continue with classes until the June recital

Upon receipt of the forms above, Juliana's Office Staff will call or email you to schedule payment. At that time you can turn in this final form filled out for office use. DO NOT EMAIL THIS FORM.
